Abstract

The Case Manager model of care, if applied correctly, can actually have a positive impact on the quality of life of patients, through a considerable improvement in the quality of care and a global patient care. It deals with a nursing approach that can be applied in a variety of different and numerous care settings, including the Mental Health setting: it is characterized by patients with mental disorders, which are characterized by behavioral and psychic alterations. Mental illnesses severely damage the quality of life of patients and provide them with severe psychological and social distress, often leading to exclusion and isolation from society. These illnesses, due to their complexity and variety, require a multidisciplinary approach, involving close collaboration between several different professionals, who place the patient at the center of the treatment pathway and have as their main goal his well-being and the improvement of his conditions. The multiprofessional team is managed and coordinated precisely by the nurse Case Manager, who is the main contact person for the case and manages the care and rehabilitation pathway, starting from the takeover to goal achievement. The case manager makes use, not only of other professional figures, but also of the network of services in the area, encouraging integration and collaboration among many different entities. The interventions and goals to be achieved are initially planned and, during the care pathway, are constantly monitored and updated if necessary: this activity finds its concrete application in the PTRI, Individual Therapeutic Rehabilitation Plan, which is a rehabilitation pathway aimed at the resolution of complex sociomedical needs.
