Abstract

Cardiovascular disease is the leading cause of death and one of the most important public health problems. Acute myocardial infarction (IMA), understood as the necrosis of myocardial tissue following a reduced oxygen supply due to the occlusion of a coronary artery, is a time-dependent disease, where the earlier the reperfusion is, the restoration of the normal coronary circulation, the more heart you can save. In this study we wanted to describe the epidemiology of patients who underwent primary angioplasty (pPCI) at the Hemodynamics of the Civil Hospital of Baggiovara, and analyze, process and present the data collected, with reference to the three-year period 2018- 2020, with particular attention to the time that elapses from the first medical contact, to the diagnosis, up to the recanalization of the vessel (First medical contact to the wire). The analyzed sample (721 patients) shows that men have a more frequent diagnosis of STEMI or nSTEMI with indication of pPCI than women. The FMC to the wire <90 minutes (gold standard according to the indications of the European Society of Cardiology (ESC)) was respected in 59% of cases, observing latency times between the onset of pain and the first medical contact, not always negligible. Regarding the first phase of the Covid-19 pandemic, the data in our possession indicate that the system has held up well, maintaining the pre-covid operating standards, without any delay regarding coronary reperfusion times, but assisting in some cases, at times between the onset of pain and the first medical contact, very far from the average. The results obtained go along with the well-established scientific evidence, which shows precisely how the reperfusion treatment in acute myocardial infarction has reduced mortality from this pathology. In addition, it highlights the importance of health education.
