Abstract

Cardiac arrest represents a clinical situation characterized by sudden cessation of the cardiac pump function, which involves the absence of systemic perfusion and results in the interruption of respiratory function; without prompt intervention it inevitably leads to death. Until now, the treatment of choice in the out-of-hospital setting is represented by the use of the ALS algorithm by the healthcare staff. The paper in question aims to examine and propose a new modus operandi: in fact, the use of extracorporeal membrane oxygenation (ECMO) veno-arterial for the rescue of out-of-hospital cardiac arrest, refractory to conventional therapies, is effective in terms of survival and neurological outcomes. The following bibliographical review aims to highlight the superiority of extracorporeal cardiopulmonary resuscitation (ECPR) over conventional cardiopulmonary resuscitation (CPR), based on studies performed both, in Italy and abroad. In addition, the new procedure for the treatment of intra and extra-hospital cardiac arrest of the AULS and AOU companies in Modena is illustrated, although it is still in the approval phase. Finally, what emerges is the central role played by the nurse involved in this procedure, who must have high-level training and specific basic and advanced skills, as well as relational.
