Abstract
mptom management and assistance in the terminal phase of the disease are highly complex for the healthcare team. In this paper I will deepen the management of delirium, pain and dyspnea, three symptoms that increase in the terminal phase of the disease. Moreover, I will treat sedation as an indication for the management of refractory symptoms, investigating the features relating to it, the type of drugs used and the importance of support for the client and family members in all the phases that characterize the final stage of the disease. In this regard, an in-depth search of the literature was conducted, through the consultation of various databases (PubMed, UpToDate, CINHAL), scientific articles and documents acquired through Google Scholar, various textbooks and from the Italian Society of Palliative Care. Several keywords were used for the research: nursing, palliative care, sedation, refractory symptoms, pain, dyspnea, delirium, setting. The main methods of managing and evaluating delirium, dyspnea and pain have emerged, along the fact that the most widely used drugs for induction and maintenance of sedation are midazolam and morphine, which induce the relief of refractory symptoms. Furthermore, it appeared that the nursing role is of fundamental importance for the evaluation of symptoms and for the support of the patient and his family, helping to improve life's quality and symptoms relief, while providing a valuable role within the multidisciplinary team that characterizes palliative care.
