Abstract

The community nursing care is mainly addressed to people with chronic pathologies, in the different phases of life, both in situations of disease and absence of it, through programs of care, health promotion and disease prevention in a territorial context. We wanted to describe a proactive care mainly developed in the territory in the various care settings, as part of an evolving Primary Health Care, with a focus on the House of Health in the district of Modena, in the specific context of the person suffering from arteriopathy of the lower limbs. This disease affects 12% of men and 15% of women limiting their autonomy, increasing the risk of limb amputation and the mortality, with an increased prevalence and harmfulness in the case of diabetes. This disease can manifest itself in an acute or chronic form and the coronavirus has worsened its effects, causing a decline in the effectiveness of the management of care processes, in the organization of nursing care as well as in the manifestation of needs by the individual. The main clinical aspects of peripheral arterial disease and the connections with the current pandemic have been exhausted, highlighting some aspects of nursing care, such as the reduction of access to the emergency room due to the fear of Covid-19 infection, while running the real risk of limb amputation. The role of the community nurse is to capture the need and convey it into the appropriate care pathway, especially in the period that we are living.
