Abstract

Article 5 of Law 219 of 2017 provides for the possibility of defining a shared planning of care that protects the right to life, health, dignity and self-determination of the person, this legislative indication is implemented through formal and structured moments interview like the Family Conference. This is the meeting between patient, family and a multi-professional team to discuss the wishes and expectations of the patient and family on the treatment path. Through the experience of a Family Conference, this thesis work aims to give an overview that can highlight the differences between the guidelines indicated in the literature and clinical practice. First, the concept of palliative care will be explored from a historical and regulatory point of view, then the palliative care network will be described, in its related forms, the different care settings and the multidisciplinary team. Finally, the tool to implement this shared planning will be examined by observing the implemented Family Conference and the guidelines in force.
