Abstract

The thesis deals with palliative care at the end of life. In the health and welfare field, the interest in this area has gradually increased over time and its importance has been underlined by national and international regulations and indications. The paper consists of two parts: a compilation study and a Case report. In the first part we can find an initial premise on the main issues, while the second part illustrates the development of a nursing care plan. The aim of the work is to know the main nursing assistance aspects that concern the end of life. It also suggests investigating the importance of narration and storytelling as a possible way, for the patient, to express her emotions and desires while she is still conscious. The first part is about the role and training of palliativist nurses in different contexts and age groups, the “Liverpool Care Pathway model”, the palliative care at home and in Hospice, the terminal phase, palliative sedation and finally focuses on the delicate role of the family. In the second part, the Case Report illustrates the study of a clinical case, focusing on the emotional and autobiographical sphere, through the Patient’s Agenda and a personal Diary. The following databases have been used to search for information: Pubmed, Cochrane and CINHAL. The key terms used have been: “palliative care”, “home nursing assistance”, “end of life care”, “family caregiver”, “home care”, “Hospice”. Normative indications provided by the SSN, the ISS, the Italian Society of Palliative Care as well as the Deontological Code of the nurse were consulted.
