Abstract

Diabetes is one of the most common chronic diseases in the world and, because of the effects that causes on the on people who have it, it can be considered as a public health problem. One of the most debilitating complications of diabetes is the diabetic foot, which, unfortunately, effects more and more people every year. Without considering the high health cost, the most important thing is the severe impact on the personal life, effecting social, physical and psychological aspects. The diabetic foot is a condition that, because of the anatomic and functional alterations caused by infections, neuropathy and ischemia, can lead to amputation. The management of the patient with diabetic foot requires an holistic care that can be pursued by a multidisciplinary team with the creation of a clinical pathway .The nurse has many roles regarding patients with diabetic foot, that include the treating of the wound and its prevention, teaching the therapeutic adherence and the self care and in some cases can lead the team by being the case manager. In this thesis I analyze, through a case report, the importance of the clinical pathway when it comes to diabetic foot and the potential nursing assistance using the taxonomy NANDA, NOC and NIC to demonstrate how an integrated care model is the turning point to achieve positive health outcomes.
