Abstract

This study aims to analyze the problem of overcrowding in emergency rooms around the world and report the solutions that are adopted today to solve or reduce the problem in particular regarding pediatric ED. Among the general causes we find: the progressive aging of the population, the number of complex patients, the slowdown of the throughput, the difficulty in hospitalization due to unavailability of beds, the extension of the duration of hospitalization and the insufficient diffusion of the Diagnostic-Therapeutic Assistance Pathways (PDTA) within hospital and integrated between hospital and territory. As regards the pediatric ED, the main causes are: poor training and excessive parental apprehension, lack of trust in the family pediatrician, lack of a pediatric medical guard. The solutions adopted today are: fast-track, new triage models, assistance structures outside the hospital, in particular for children, See & Treat, OBI, primary care and above all parent education. The application of these solutions has brought important improvements to the overcrowding in the ED.
