Abstract
This thesis aims to highlight how the home visit and a personalized project have a positive impact on the patient with mental disorders and on his network (family, social life, work etc.). The domicile becomes the most suitable place where to meet, know and treat the patient; in this way, he feels freely allowed to express preferences, ideas and feelings about his condition and his recovery path in a place he considers “safe”. This thesis focuses on the importance of a multi-professional approach, which implies taking the person in charge on his totality, in a holistic way. Moreover, in this case, the nurse’s professional and relational skills become a tool of care. It’s described how the key role of the nurse is to choose suitable interventions compared to the needs of the person and his family unit. The purpose is to begin a process of changing which will help the patient to improve his health and welfare, living in a self-directed way, at the very best of his chances.
