Abstract
The purpose of this paper is to highlight the need to apply a patient-centered care model, therefore on the continuity of the cure. This thesis analyses changes that occurred in the nursing profession and it also studies literature in order to comprehend in a better way the nurse's role in the complexity of care and, especially, in transitional care.
Nowadays the prevalence of chronic diseases has sharply increased, creating innumerable cases of complex and multi-pathological patients who felt the need to modify the provision of assistance. The continuity of care represents a goal and, simultaneously, a strategy to respond in a more effective way to this problem. In fact, by definition, chronic patients are incurable and they must be treated for long periods of time, getting in touch with various professionists. In view of the above, the assistence had to evolve, in order to move with the times and the new needs of the society; it felt necessary to introduce the "initiative medicine", a model that includes a global handling of the patients, acting with proactive logic that aimes to promote the absolute well-being of the person and no longer focuses on the mere care of the single pathology. The thesis also underlines the need to coordinate the performances of the various professionists and services, recognizing the integration of care as a turning point for the improvement of the Health System.
