Abstract

Background
Chronic diseases are the main cause of the burden of disease and death in almost all over the world. Healthcare organizations need to improve the skills of health professionals, focus on prevention and provide planned care for predictable complications.
Purposes
This thesis aims to deepen the Spanish health strategy for chronicity, then study the organization of primary care and the role of the Family Nurse, then evaluate the respective Italian situation of the Health Houses and of the new figure of Nurse Case Manager.
Materials and methods: The survey is conducted through a review of the literature by consulting the Google Books, BiblioMO, Opac SBN, Medline / Pubmed and Lilacs, Cochrane, Elsevier databases and various internet resources.
Results
From the analysis of the two contexts, Spanish and Italian, it is clear the usefulness of a figure that follows the person throughout the path of illness, ensuring that his health needs are met, establishing a close relationship with the patient and the patient. family, acting as a liaison between the patient and the various health services and ensuring compliance with the established care plan.
Conclusions
The Nurse Case Manager is a figure most suited to providing quality care and greater continuity of care for patients suffering from chronic disease.
