Abstract
Home dialysis in recent years is proving increasingly beneficial for people with chronic renal failure, including improved quality of life. This factor has been poorly evaluated in the past, not considering the fact that it represents the key to the success of dialysis therapy. For this reason in recent years there has been an incentive for this method in order to better treat chronic diseases, which incidence is always higher in the population. Through the elaboration of this thesis the importance of the figure of the nurse case manager is demonstrated through a process of education, both in the pre-dialysis phase and at home, aimed at making the person more aware of the various dialysis methods, making it part active in the process, with the aim of encouraging the use of home dialysis.
