Abstract
In modern society chronic diseases represent a very common and growing phenomenon, where population approach around an increasingly high average age and an ever greater request for assistance and health services. This causes a change of person services, strategic methods for patients and caregiver involvement by a new action and communication setting. This composition talks about the chronic degenerative diseases management and it proposes an analysis of care needs, In particular of multiple sclerosis patients. In order to guarantee a quality care for chronic patients it is necessary to develop initiative medicine and assistance thank of the promotion of new territorial Nursing Services or the fortification of those already present, in an integrated way. It has been focused the development of new nursing figures that are emerging on chronic patients, in an ancient society that needs caring. Through the cooperation with the other professionals of the team and the advancement of clinical pathways, the nurse ensures a global and shared care of the patient and his family/caregiver. Moreover, great importance has had skills increase in chronicity and about different care methods, in some context already applied.
