Abstract
In the Rehabilitation Medicine O.U. of the “Ospedale Civile di Baggiovara” (MO), there was the opportunity to modify the nursing file, in order to make it a tool facilitating the introduction of the new model of Primary Nursing. The study focuses on the execution of a complete initial assessment according to the 11 functional models of Marjory Gordon, and on how much this new method of care leads to the well-being of the person. Primary Nursing is therefore born from the idea of caring for people as everyone would wish for himself and places a great role of responsibility in the nurse who cares for the person. The thesis is composed by a first introductory part where were described the importance of initial interview, and the 11 functional models of assessment of M. Gordon. Subsequently we will procede with description of the case histories of people hosted by the O.U., and their nursing needs, together with an exposition of the new model of Primary Nursing. During the last chapter we will explain the project, which consists in planning nursing diagnoses, selected according to the standardized needs of patients through the use of the NANDA-I, NOC and NIC taxonomy.
