Abstract

In recent decades, the health situation in Italy has changed in several respects, mainly due to the economic crisis and the aging population. The National Health Service has tried to make some improvements, including the decentralization of hospitals and greater personal assistance, interventions that translate into greater investment in the structures and services present in the territory. With greater autonomy for health professionals, especially nurses, they are able to provide better personal care. The fusion of these two important changes is evident in new structures called Community Hospitals, which serve as a link between the hospital and the territory, trying to reduce unnecessary admissions as well as the length of admissions after the acute phase; this is combined with mainly nursing the patient, obviously in collaboration with a team of health professionals such as doctors, physiotherapists, OSS ... In these structures, patients with chronic recurrent problems, or patients with difficulties recently discharged from hospital, for a certain period can find constant observation and health care, thus giving the patients and their carers time to get used to the new health conditions.
After describing the new healthcare context, this project analyses the characteristics and requirements of the new structures, focusing on the professionals who work there (mainly nurses) and the assistance provided.
Finally, the Castelfranco Emilia Community Hospital was examined, comparing and evaluating the welfare model applied with the standards defined in national and regional regulations, as well as literature references.
