Abstract

Diabetes si one of the most common chronic diseases in the world and represents a significant problem of public health, as it has a strong impact on the lives of those affected by it. One of the most disabling complications of diabetes is the "diabetic foot". More and more people each year present this complication that, in addition to being associated with a high health cost, has a devastating impact on the persons life quality, affecting negatively both physical, psychological, economic and social aspects. The diabetic foot is a syndrome where neuropathy, ischemia and infection conduct to anatomic and functional alterations that can lead to amputation. The management of patient with diabetic foot, given its care complexity, requires a holistic approach and a global assessment of the person by an inter-professional and multidisciplinary team. The nurse has an important role not only in the treatment of the diabetic foot, but also in its prevention, and especially in therapeutic education, promoting the empowerment of the person, the adherence to the care plan and the ability to self-care. In this thesis is analyzed a clinical case and is planned the nursing assistance using the taxonomy of NANDA-I, nursing results NOC and nursing interventions NIC, to demonstrate how the nurse, using an integrated care model, is a winning strategy for achieving positive health outcomes
