Abstract

More and more initiative medicine, with services that meet citizens, more and more concrete integration between professionals, continuity of care with the hospital and community participation. In this direction, the Health Centre in Emilia-Romagna goes. This means that the taking charge of the person will take place with paths that bring together different professionals and services. A process already in progress, in fact, in many situations, but not yet uniform throughout the region. Compared to the advice provided by health personnel regarding changes in lifestyles in Emilia-Romagna, the situation is similar to that found at national level: on average only 1 in 2 among people who have risky behavior, receives advice to change them; in the case of alcohol consumption, this proportion is even 1 in 6 (PRP 2015-2018). The aims of this thesis are two, that is, to perform a wide and detailed overview of the state of the art of Health Centre , in light of the recent regional provisions of Emilia Romagna and introducing the main concepts of the Transtheoretical Model of change of DiClemente and Prochaska, necessary to support health professionals in the management of the process of changing the lifestyles of the population. Bibliographic research was carried out by consulting national and regional websites, regional reports, textbooks, and decree-laws. We are hoping for an increasing collaboration and sharing of objectives and actions among all the protagonists of the Health Centre, to realize that horizontal integration which is their real added value. With regard to the prevention and promotion of health, particular importance must be given to the development of the professional skills of health workers, and therefore also of nurses, in managing the change in lifestyles.
