Abstract

Rational of the study. The extravasation of chemotherapeutic drugs needs attention especially by nurses, who are the first to intervene in case of an adverse event. Due to continuous increase in available drugs, the guidelines used at the Oncology Center of University Hospital of Modena have been revised and updated with new chemo-immunotherapies. Finally, a statistical analysis of incident reports was performed, in order to assess whether the introduction of new therapies was associated with an increase in their extravasation.
Materials and methods. Using the COMNET computing platform, we screened the chemo-immunotherapeutic drugs recently introduced and currently in use at the Oncology Center not included in the current guidelines. Drugs were classified according to the risk of extravasation. For the calculation of events we have analyzed the incident reports data from 2014 to the first semester. Finally, we have proposed a new classification of drugs, in order to facilitate their management.
Results. We have identified 15 new drugs used in clinical practice which were not included in the extravasation management card. On the basis of the technical sheet, we have classified them and properly reported in the new classification proposal using a colorimetric subdivision in relation to the severity that each drug class can give in case of extravasation. The statistical analysis has shown, however, that over the years the classic chemotherapeutic drugs were more frequently reported in the extravasation compared with the immunotherapeutic agents, and that the extravasation/year incidence was rather modest.
Conclusions. Although, as shown by the study, the incidence of chemo-immunotherapic extravasation was modest in the years studied at the Modena’s Oncological Center, however it remains a potential danger for patients and is subject to nursing responsibility and the guidelines need continuous updates due to the constant increase in new therapies and clinical trials.
