Abstract

Psychiatric Intensive Treatment Facilities (PITF) are health inpatient settings for patients affected by psychiatric diseases with impaired personal and social functioning. The purpose of this study was to analyse some variables of patients hospitalized in a PITF, in order to highlight how they correlate with clinical improvement, evaluated through the Health of the Nation Outcome Scale (HoNOS), and the duration of stay. We retrospectively collected demographic and clinical variables related to patients and their stays in the PITF “Solida Trasparenza” of Az-USL in Modena (catchment areas of 520,000 inhabitants) from 1-11-2016 to 31-10-2017. We statistically analysed the comparisons between them. In our sample (106 patients), males (n=51) were followed by multiple outpatient services and lived with their origin family, whereas females (n=55) were treated only by Mental Health Service (p=0.048) and lived in their acquired family (p=0.015). The PITF stays (n=128) were divided into two groups according to the median of duration: ≤29 (n=83) and >29 days (n=23). Patients hospitalized for >29 days had more frequent insufficient economic conditions (p=0.027). The most common nursing diagnosis NANDA-I was "Impaired mood regulation" (p=0.018), whereas "Self-neglect" was associated with an increase in the duration of stay (p=0.003). Other variables represented significant risk factors for extending the duration of stay at multiple linear regression: "many medical consultancies" (p=0.001), "insufficient economic condition" (p=0.045), "clinical interviews extended to institutional figures" (p=0.013). At discharge, a high percentage of patients were sent home and presented significant clinical improvement at the HoNOS in comparison with the admission (p<0.001), greater in women than in men (p=0.018). This survey confirms the effectiveness of therapeutic programs in PITF to achieving the acquisition of personal autonomy needed for independent and non-institutionalized life.
