Abstract

This work, an attempt has been made, to understand what is meant by advanced competence, to try to get into the concrete and prevent an ideological debate, but above all, it want to clarify that which has been reported in the past recent years, the right for nurses to prescribe; specially the prescription of medical products or devises and pharmacological prescriptions, in Italy. To meet this objective, we analyzed a corporate Protocol drawn up by the Retirement Home IX gennaio in Modena with the aim to address the need to ensure the proper use of a therapeutic prescription in terms of "need" and and a recent Cochrane review comparing the results obtained from a non-medical prescription to that of a medical one. Furthermore, it also analyses the legal and deontological responsibility of nurse prescribing in Italy. To meet this objective, reference has been made to a number of data and information sheets.  To understand what is specifically meant for advanced competence, reference was made to a study conducted on a group of nurses from different wards and with different work experiences, asking to describe in which situations you would have turned to a specialized nurse and list the skills, which in their view, a specialized nurse should possess.  The result which emerged from this study was a certain diversity of perception and interpretation regarding current practices. To comprehend how the training system responds to the demand of this type of competence, reference was made to an analysis of training for nurses of the academic year 2015-2016 in Italian Universities. In international reality, numerous studies show that nurses which possess advanced skills improve patient outcome, however advanced figures are numerous and at the same time quite varied.
In closing, many reflections have been made regarding the transformation of roles of healthcare professionals and the impact that this has on the limits between professions.
