Abstract

In this elaborate thesis is developed type 1 Diabetes Mellitus and the motivational path necessary for the people affected by this pathology, so that they can find in them the desire to engage in self-management. In particular, the obstacles that the person can develop against the disease have been analysed, trying to identify the applicable interventions to increase their compliance, including the use of new technologies and devices. I took the decision to take this argument on the basis of my sister’s personal experience that saw my sister as the protagonist of this illness. My inability to relieve her from this burden has led me to look at the topic, trying to figure out what interventions are needed to encourage self-help and good empowerment in the person. In addition we want to describe the importance of nurses counselor to support people affected by this pathology in adolescence so that they can find the ability to react, despite suffering and initial non-acceptance. The method used in the realization of this thesis includes a bibliographic research based on scientific literature by obtaining documents, guidelines, and articles from databases such as PubMed and search engines such as Google Scholar and Google Books. In addition, an on-site observation at a diabetes service was planned, where the transition team plays a key role in the global intake of the young diabetic , through nursing counselor and the development of personalized care plans.  It has been evident that professional nurses are a leading column in accompanying young diabetics to raise awareness of their health,  by leveraging the potential of the individual and the family through team work.
