Abstract
Nowadays, chronic diseases represent a menace to a big part of the population, towards a middle age that is getting higher and higher. In my elaborate it will be described how to cope with this problem. With this I mean, through an initiative medicine that has important goals, as the promotion of the “Houses of health” which have a growing presence in Emilia-Romagna, and the development of a new profile of our profession, the “House manager male nurse”. The latter that I mentioned, through its own role dimensions and its own instruments (for example the therapeutic education) and the of a ICP (integrated care pathways), collaborates together with other professionals therefore they work in an inter-disciplinary team, and all this with the aim of optimizing integration, coordination and the continuity of support to the patient.
