Abstract
In the last few decades, the progressive increase in the population's life expectancy, the simultaneous increase in chronic illnesses and the current economic crisis have led to an increase of needs of assistancei. To this purpose, organizational changes have been implemented, incorporating the model of the "Community Hospital" a new district resource between the traditional hospital and the territory to meet the health and social services needs of a predominantly elderly population. This work aims to acquaint the community hospital, and the role of nursing case manager and how this figure can create a strong relationship with the patient becoming the reference figure for the patient, family and / or caregivers and other health and social workers, allowing the patient to take an active role in the welfare process, as he is empowered and educated to understand how much he can do to improve outcomes and quality of life.
