This paper shows the way to get a research protocol from a question generated during the clinical activity, beacuse “The search starts from the bed of sick person” (P.di Giulio). This research aims to assess whether the introduction of Aree Vaste, and specifically the unification of the territorial system 118 as responsible for Modena area has led to a change of health outcome for people. We were chosen SCA and Stroke disease subject to early treatment. The research's step were question formulation, retrievel of literature and available data sources and the subsequent critical evaluation. The core of the work was that of building a retrospective observational research protocol to compare between their two patient cohorts ( before and after the establishment of the operational center Emilia Est in 2014). Population under consideration is all patients included in the Modena province that have turned to the 118 and for which early treatment of SCA and Stroke during the periode under review. The sample il hypothesized in 3500 patients whit SCA, treated with primary percutaneous transluminal coronary angioplasty and 220 patients with Stroke treated with thrombolysis.
