Abstract

Weaning from mechanical ventilation is the important process that leads towards complete recovery of spontaneous breathing. The weaning process itself can require up to the half of total time spent by patient in mechanical ventilation condition. Nurse figure is present more than clinicians throughout hospital stay, so they can play a significant role in weaning process management. The main objective is to review classic weaning compared to new techniques in course of studies.
There were selected some topic-books published in last 5 years, and articles published on the main online-journals between 2001 and 2017. The exclusion criteria was absence of full-text article, or limited relevance with principal topic. The research was made using the online resources, such as: PubMed, Medline, Cochrane, Google Scholar and ResearchGate. There were examined  studies containing RCTs, systematic reviews and surveys. Pediatric population was excluded. Keywords: Weaning, mechanical ventilation, nursing, critical care, ICU ventilation.
There were identified main problems: “weaning readiness” indexes that are commonly used for patient evaluation are constantly updated; however none of these are 100% effective; use of many indexes increases the probability of success; use of new techniques makes process safer. Nurse's assistance has been proven very important for functional recovery. Furthermore, it was underlined the necessity of nursing involvement in decision-making process. Use of protocols for nursing management seemed to improve nursing involvement, but it was also seen as a limit to intervention.
A continuous development of new medical practices, should enhance also nursing involvement regarding decision-making, and the number of researches on nursing, regarding respiratory weaning, to ensure better care process.
