Abstract
The phenomenon of overcrowding in emergency department (ED) is a very timely event and increasingly growing. These conditions generate long waiting times before receiving the medical examination. The long waiting times are considered as an empty moment, mainly because the patient does not perceive a real taking charge by the health service.
The long waiting times also contribute to structure the belief, among the citizens, that there is a general lack in facing the health assistance request.
Over the years, we have witnessed the birth of care pathways, such as Fast Track and See and Treat, whose objective was the reduction of waiting times. These care pathways are mainly targeted at patients who present in the first aid with minor health problems that can not be cataloged as real emergencies, and for which it often exist a solution at a local level.
The aim of this paper is to propose a strategy to reduce the permanence times within the emergency department, for those patients classified, during the triage, as green codes. The goal is to intervene on waiting times, often incompressible, turning them into real active moments, in which the patient is structurally taken in charge of the Health care service. With this purpose, at the OCSAE of Baggiovara, it was born the figure of the senior nurse, which performs a new function: the post-triage.
The practitioner takes immediately care of the patient right after the color-code assignment. By other words, he ensures the implementation of diagnostic and therapeutic care pathways, and, at the same time, he identifies the needs and provides custom responses. Nowadays it is possible to talk about active waiting meaning all that is configured in real welfare activities, programming and revaluation.
With the implementation of the figure of the senior nurse, it would be possible to affect the processing times by effectively reducing them.
