Abstract

Cardiovascular diseases are nowadays one of the major health and economic
problem all over the world.
The major cardiovascular risk factors are modifiable as mostly related to
improper lifestyle.
In this context, the role of the nurse may prove crucial. Using relational skills
like counseling, health promotion and therapeutic education, healthcare
professional focuses on the patient care process. In this process the assisted
person may be accompanied in a path of awareness of its potential and
resources in respect of its psychological and physical well-being.
Nurses share their knowledge with the person, recognizing its uniqueness in
the path of prevention, care and treatment of diseases.
The patient may become aware of the risk factors and the various ways to
change them and neutralize the effects.
The choice to develope a care plan on a real patient, wanted to demonstrate
how theoretical guidelines and recommendations, may prove effective in
clinical practice.
For the planning of the nursing healthcare plan, taxonomies like NANDA-I,
NOC, and NIC were used
