Abstract
The aim of this thesis is to make people understand the importance of primary prevention in cardiovascular diseases. These diseases are still rising with a consequent increase in morbidity and mortality. Emilia-Romagna has extended a primary prevention of cardiovascular diseases Project called "The integrated interpretation of cardiovascular risk in Healthcare Homes", which refers to the Regional Prevention Plan. The purpose is to remove the modifiable risk factors in healthy people in order to reduce the development of these diseases. The population considered is men of 45 years old and women of 55 years old; is the age at which the risk of cardiovascular disease increases. The best setting for the implementation of this Project is considered to be the Healthcare Home, a reference point for the citizen where the user can be taken in charge to follow his path. The survey methodology used is retrospective study, related to Bomporto and Cavezzo’s Healthcare Homes. From the results obtained, it was possible to perceive that the Project is in continuous development, a necessary condition to achieve improvements both in terms of instruments for the implementation of the Project, and in terms of results obtained.
