Abstract

This thesis aims to be a starting point and a personal consideration about the  management of chronically and terminally ill patient. It arises from a direct experience lived during the internship at the unit of the Domiciliari in Carpi. Has been sought to more thoroughly investigate the origin of discomfort for both sympathetic and professional sides, experienced by the patient's family and me. The purpose is trying to define the starting points and the possible improvement for the experience, however damaging for the both parties. This paper’s main focus is to investigate if there is, beyond the clinical practice, a way to better manage this condition for the patient, the family and the medical staff. Starting from this target we tried to define the Welfare State, the path and the people in charge of the National Health Service, as well as the needs of the patient and, above all, the Nurse skills. During the writing of the thesis, it has been chosen to maintain an observing position, which allows to identify organizational and personal weaknesses. It was also sought, starting from the detected problems, to think about what proposals could lead to an improvement. The thesis has been structured to try to understand if there is the possibility of offering, from the outset, an early assistance to the terminally ill patient. It has also sought to understand, whether it is possible, to manage in early and preventative manner, of what could be the path to the end of life in the most respectful way and with the acknowledge of the patient. The topic will be analysed from the point of view of a student who attends the Degree Course in Nursing and it will assume that Nurses are the professional figures who are found to share highly emotional situations with the patient, whose requests are not limited to the mere professional skills but mainly to the personal presence of one who is taking care of them.
