Abstract
Cost containment has been a challenge for health sistem for a long time.
This degree paper presents a literature review, in order to seek solutions with organizational and territorial models that can contain an increase of re-hopitalization costs.
By examining heart failure as one of the chronicle pathologies involved in re-hospitalization, a real increase of resources destined to chronicity was highlighted. Technical abilities and nurse's educative interventions in collaboration with other experts, can represent a key point for cost control and better results in matter of patients' health. This gains a particular importance expecially in health systems that focus on replacing human and instrumental resources.
