Abstract
The aim of this thesis is to find the best evidence in the scientific literature to deliver personalized and quality nursing care to patients with heart failure (HF). The paper describes and classifies the disease, its clinical manifestations, drug and non-drug therapy. It analyzes the patient's course of treatment, focusing on cardiac rehabilitation and the phases that compose it. The aims is to highlight the important role of the nurse about the total care of these patients, with a particular attention about the therapeutic education, as a tool to allow to reach the maximum degree of wellness. Because the assisted’s conditions change with the progression of the disease, the nurse must make a continuous assessment and consequently change the planning care. To do this it was made a nursing personalized planning care with taxonomy NANDA, NOC and NIC, considering a patient with heart failure, overweight and with smoking status, which according to the signs and symptoms could belong to Class II, according to NYHA (New York Heart Association).
