Abstract
In this script, after a careful analysis of selected articles, were found out many problems in providing care support to patient with chronic heart failure. In particular there is the necessity of an effective information exchange between health professionals (doctors, nurses, specialists), a way that allows a constant patients’ follow up in their disease and the selection of specific questions to create a personalized management (home care, dedicated ambulatory, etc…). The figure of the nurse turns out to be essential in the management of these patients in order to reduce hospital readmission and therefore in the prevention of chronic disease acute exacerbations. Care activities provide a global management of the patient and his family with self-care education interventions and monitoring signs that allow early recognition of symptoms that could lead a worsening of patient’s clinical condition.
