Abstract
This thesis aims to analyze the figure of the Case Manager Nurse inside Emergency Departments, in particular in the emergency room, context in which it is poorly implemented, but where it can lead to very positive results in terms of assistance quality for patients and their families, taking in consideration the necessity of cost containment and optimization of available resources, which are essential aspects in this historic moment.
The first chapter presents a historical overview of the ICM origins, then highlighting identity , role and responsibilities. In our country, this figure was already long inserted in various care settings, but has a very low development in Emergency Departments. Therefore in the second charter this elaborate presents other international application experiences of that model in these departments. The third chapter highlights the advantages of inserting the ICM under the Italian emergency room, in particular, putting attention to the elderly population as "repeated consumer of health care".
The conclusions emphasizes that the elderly represent a significant percentage of users of the emergency department , which became crucial interface between hospital and territory , and where the care continuity should be activated. The professional nurse has become the most appropriate figure for the Case Manager role, thanks to his multidimensional education , his relational skills, organizational knowledge of the healthcare context in which operates, and functions of the various health professionals to whom they can address.
Case management is a methodological approach that offers to parients an effective way, that avoids unnecessary medical performances, and determining for the National Health Service consequent economic savings by an optimization of appropriateness and effectiveness of care provided.
