Abstract
In the last few years, working in a network (hub and spoke) in which different actors face a common cause together, sharing goals, is becoming popular. The organization of medical transport has grown in complexity and size such that local health agencies are required to develop management systems with specific expertise and dedicated resources.
Patient management during procedures that require a transfer can change the results and prognosis by about half. It is important to know how to handle the various difficulties and adverse events that may occur. Transport (inter-hospital and in-hospital) is now a moment of dedicated assistance, especially for critically ill patients.
There are time periods and criteria that must be respected. Furthermore, during transport, physiopathological alterations cause acceleration / deceleration, noise, vibrations, and altitude changes, each of which change the clinical status of the patient. This is also why it is important to define the minimum requirements of the equipment required for transport both within and outside the hospital, as well as the right crew and clinical standards and technology necessary to reduce the risk of deterioration of vital signs.
The literature states that wider use of standardized classifications, control checklists, guidelines and training plans appropriate for the teams are required in order to enhance safety and lower risk during transit and in the long term. The use of nursing teams with training and special preparation has already produced good results in Anglo-Saxon (English speaking) countries; a good starting point for the Italian reality would be to elaborate plans to transfer based on the resources available.
The basic criteria proposed for the transport of critically ill patients also focuses on three categories of particular patients: traumatic, neurological and neonatal.
