Abstract

The aim of the study is to observe if the patients assigned to the Medicina Riabilitativa Unit of the Nuovo Sant'Agostino Estense Hospital (NOCSAE) in Modena have enteric colonization by Klebsiella pneumoniae at admission. The hypothesis of this study is that these patients, during their hospitalization, restarting the normal feeding per os, turn out to be negative at the screening with rectal swab for KPC. The massive spread of Carbapenemase-Producing Enterobacteriaceae (CPE) and, in particular, Carbapenemase-Producing Klebsiella (KPC), is a growing public health problem because it causes infections difficult to defeat and it’s related to an high mortality rate. The therapeutic options are limited; only the prudent adherence to policies of control and the prevention of infections is able to contain this phenomenon. Therefore, it is necessary to find new effective strategies. A qualitative observational retrospective study with no participant observation has been conducted. Data were collected and it was decided to start an initial cluster analysis of infectious cluster of the year 2015. It was selected a sample which met the criterion of positivity for KPC at admission. Data processing has shown that 60% of patients, fed enterally or parenterally, were dismissed with positivity at screening with rectal swab for KPC, confirming the study’s hypothesis. 20% of them, despite being fed by mouth, is released with positive swab. A further 10% is released with negativity at screening with rectal swab, despite being fed by enteral or parenteral nutrition. Only a 10% of patients fed per os, is released with negativity at screening with rectal swab for KPC, as from the study hypothesis. However, it is not clear if there is a real correlation between feeding by mouth and negativity at rectal swab. Therefore, we recommend further prospective case-control study
