Abstract

In 2009 in Modena, following a training need expressed by territorial medical rescue 118, it had been set a course aimed at teaching treatments of patients who have tracheotomy / tracheostomy: the TRAM (tracheostomy Airway Management) concerning tracheotomy patients care in emergency situations. This training need stemmed from an increase of number of this type of patients in the territorial area, and consequently the increase in requests for help to 118. Therefore, the SICU (Surgical Intensive Care Unit) of the Nuovo Ospedale Civile Sant'Agostino - Estense Baggiovara (AUSL Modena), took charge of this question and created a special program for the Modena’s relief operations centre. This course, which is still active, is divided in a theoretical and a practical part. The theoretical part is about everything that concerns the tracheotomy: the modification of the anatomy and physiology of the trachea after the operation, the emergency management, as well as the devices associated with it. The practical part, however, focuses mostly on the procedures and manoeuvres to be performed, using exercises and role plays. At the end of this journey, lasting 5-6 hours, it is distributed a test to verify the learning of knowledge. The same questionnaire has been administered again at a distance of time, in order to verify the maintenance of such knowledge. After explaining all the contents of this project, we have studied collected questionnaires. The data analyzed show that, within the team working in Modena's relief operations centre , knowledge have been maintained, having obtained, specifically, 81% of correct answers, and only 19% with negative feedback. It is hoped to apply the TRAM in a wider field.
