Abstract
This path thesis aims to show how the increase in chronic diseases, such as cancer, heart disease, stroke, chronic respiratory ailments, but especially diabetes requires a greater commitment of resources, economic and welfare to our patients. The Chronic Care Model will be our solution; this model, designed by Edward Wagner in Seattle underlines the importance of the patient to become an active protagonist of his disease to enable him to know the potential complications, because it makes him more responsable and it allows to identify the first signs of a complication such as anxiety and fear that not always activate a positive behavior. An example tangible of this model is the House of Health in Finale Emilia, which provides services and informations to all citizens, including patients with diabetes mellitus type 2 through the application of a path of integrated management multi professional. The model of the integrated management of chronic pathologies provides, starting from the definition of the target population of patients , their stratification or classification of patients in sub – target on the basis of the degree of evolution of the disease and specific needs deriving therefrom and from which it originates plannings and diversified strategies to follow up.
