Abstract
In this elaborate I do a review of the literature on Assistance Nursing Home that you can lend to a patient suffering from COPD. In the first part of elaborate i define the features of COPD explaining risk factors, signs, symptoms and social costs of this disease. In the second chapter I the question of Territorial assistance you, describing the regulations that govern and characteristics of Continuity of Care. In the third chapter, I describe the activities of home nurse's, which not only carries out technical assistance-practical, but, above all, apply his knowledge and skills in the part of an Integrated Home Assistance and cooperates with other professionals in the Multidimensional Assessment of the patient. The nurse of the third millennium must be able to work in a multidisciplinary team, able to satisfy the increasing demands of health needs. In chronic conditions inevitably affect the lives of the patient's family and in this thesis examines their roles and support that nurse can give to involve them in the process of care.
