Abstract
Palliative home care provided by inter pares team allow the continuity of care for terminal cancer patients, also providing support to families and caregivers and thus creating a coordination between the various sites of care.
It is made up of doctors , nurses and other professionals who need to acquire specialist skills in palliative care to meet the needs of sick people , defining goals of care that include not only the physical, but also psychological , social and spiritual .
The Model of Supply Widespread Palliative Care and End of Life is an important tool for integrating organizational services at the local level and a resource assistance .
