Abstract

This thesis is about nursing care to the patient with polytrauma. In particular will be thorough transfusion therapy, in its management and application in this tyoe of critical patient, examinig the procedures in the light of the protocol for the management of massive transfusion PTM. This protocol is present at the operating unit of emergency departement of the New Civil Hospital Sant' Agostino Estense Baggiovara. While there is a vast literature on the subject of blood transfusion and more generally its derivatives in emergency in polytrauma patients, this type of protocol was developed to adopt the best strategy in the event of patient hypoperfused, which must be subject to massive transfusion. The objective of this procedure are the reduction of the time of activation of the Transfusion Centre, and to reduce the use of rare groups readily identifying the type of the blood of the patient trequiring transfusion. The first chapter defines the polytrauma: characteristics, indicators and demages that may results. Defines the type of scales and indexes of severity used throughout the care process, and the concept of Golden Hour. The second chapter describesthe prehospital care, examining it in its two main phases. The third chapter begins to examine the Protocol Massive Transfusion, specifically its activation and application situations. Describes the triage, the triage nurse's role in the activation of ptrotocols provided by the hospital. The fourth chapter is particulary of hospital assistance, describing the procedures for stabilization of the patient, the type of diagnostic tests that are run, and monitoring of polytrauma patients in shock room. In the fifth chapter discusses the transfusion therapy and its application in case of massive bleeding.
