Abstract
During my university and my internships I aroused the desire and the need to point out how in the Italian population and especially in the region of Emilia Romagna is the key figure of the case managers and case management through which you can get to a state of welfare state best for the patient. The Nurse Case Manager ensures coordination of care along a continuum of care at the same time covering various operational roles. Clinical role is responsable for ensuring the potential/real problems of the patient and the family considering the physical, psychosocial and emotional status, implementing a plan of care in collaboration with other members of the interdisciplinary team. In my thesis I describe a real case of a patient admitted in the post acute care ward of the Policlinico of Modena. As a nurse case manager I performed the assessment, nursing diagnosis and assessment scales, the interview with the family and the family doctor where it was decided that the patient can be dimisso Villa Of Lime Trees for continue the rehabilitation and in the meantime the family is organized at home for the return of the patient at home. So early patient discharge time saving the NHS and patient's recovery after 15 days of rehabilitation will be discharged home with the objectives achieved rehabilitation that had been planned.
