Abstract
Diabetes Mellitus is a Chronic Disease with a high global prevalence. Knowledge and skills about Diabetes Self-Management make the person affected by this illness able to prevent the most damaging complications, so its represents an important goal for the entire healthcare team. Therapeutic Education is critical in making the diabetic patient able to acquire Self-Care skills. This is why the nurse must know the different tools and collaborate with all the professionals who are treating this type of patient and that are involved in the educational process. In order to do this, he should know about the different tools and professionals revolving around Health Education. Specifically, the nurse should possess relational skills with abilities in Counselling, with a particular focus on Motivational Interview and Empowerment, in order to develop a customized Nursing Care Plan for the diabetic patient and also his family and social network, aiming to get, as nursing outcome, a patient who is able to manage diabetes, its treatment and prevent complications. The nurse, as a professional that promotes health, has all the prerequisites and potential to win, along with other professionals, a challenge so demanding, such as the one represented by chronic diseases.
