Abstract

This work focuses on the importance of nursing activity in the field of chronic disease management. Its purpose is to improve the life quality of patients, relying upon the principle of therapeutic education.
The first chapter describes the change of health services designed to realization of a net of health and social services on the territory, that are related with the increasing number of chronic diseases. The purpose is to reduce the hospitalization to take care of patients directly at home. Therefore it is necessary to sustain the patient and to guide him through self-care in order that the patient can cope with illness and the interference of the disease in his daily routine. This is possible trough  therapeutic education, which will be discussed in chapter two, that explains the process of care planning. Therapeutic education is defined not only as an educational opportunity, but also as a therapeutic intervention for emotional and psychological support.
Finally, we highlight nursing skills, normed by Profilo Professionale and Codice Deontologico, that  make the nurse also responsible for the educational component of a patient.
The third chapter defines the patient with permanent enterostomy, and it illustrates his nursing management. It exposes the experience of training conducted at the stoma care clinic of Carpi from which  the idea of realizing a new method of therapeutic education with the use of a new tool; the purpose is to give a concrete example of how the therapeutic education can improve the life quality of a chronic patient.
Patients with a stoma are very frail and scared of the change they have to deal with. Nurses, with their educational, communicational and relational abilities, can help them to engage all the difficulties that the disease raises in the course of daily routine. Finally, the fourth chapter describes the implementation tool and it offers an analysis of the literature, to provide support on the basis of evidence.
