Abstract
This thesis analyzes the development and routes of care in the Primary Care Department, to linking them with the needs and requests for the region Emilia-Romagna.
After a description of a Nurse profile, his legislative story and working models, I've described the organization of work in the PUASS ( "Punto Unico di Assistenza Socio Sanitaria" the unique point for social and healt care assistence) and in the ADI ( " Assistenza Domiciliare Integrata" the integrated home assistence), to get to talk about the nursing ambulatory and the Healt Home, describing applications and sperimental projects of nursing outpatient assistence.
The nursing roles were examined by the aspects of relationship and therapeutic education with patients; there is an argumentation about the work of Case Managment and about the development of Case Manager's profile.
