Abstract
The phenomenon of chronic, due to increased life expectancy and at the same time exposure to risk factors, is clearly growing. The chronically ill patient is complex, as this often are comorbidities, in addition to the underlying disease, not to mention the impact that you have not only on the physical but also psycological, social and economic. For all these reasons the assistance and care to the chronically ill, based solely on the resolution of the moments of acute, are insufficient and inadeguate cope with all the problems. The purpose of this paper is to investigate the evidence that proved the effectiveness of the Chronic Care Model, a model that proposes the change of paradigm, focused on sharing project between the care team and patient, management in the acute stage and stability. Although the numerical range of chronicity requires further study, this model was shown to improve the management of various disease and to raise awarness of the health personnel on the need for change.
