Abstract

Introduction: Mechanical ventilation should be discontinued as soon as possible as it can be associated with numerous side effects and complications. In an effort to reduce the morbidity and mortality associated with mechanical ventilation and clinical research has focused on reducing its duration.

Objective: To summarize the best scientific evidence regarding the use of protocols during weaning . Make sure that you use them injections of benefits. Analyze the role of the nurse in assisting the patient during the process of weaning .

Materials and methods: It was carried out a systematic literature review, consulting databases Medline , Cinalh and the Cochrane Database for Systematic Reviews , search engines google scholar and google books , the website Aniarti and the magazine "Scenario ." We included all studies that dealt with adult patients mechanically ventilated for a defined period.

Results: Numerous scientific evidence shows that the use of protocols leads to a decrease in the duration of mechanical ventilation , weaning and ICU stay , but it is not possible to evaluate additional beneficial effects. The nurse not only has the task of monitoring the patient . Weaning managed by the nursing staff , through a holistic approach , based on protocols is a safe practice and in close relation with the quality of care provided . The decision-making process faced during weaning is influenced by the work environment and in Italy is usually the doctor who makes the decision on the advice of the nurse .

Conclusions: The protocols are not substitutes clinical judgment , but allow a systematic and multidisciplinary approach to the problem that achieve significant beneficial effects. To ensure the independence of the nurse in this process is necessary the support from experienced staff and adequate training .
