Abstract

This thesis aims to explore the project named “Casa della Salute”, created and promoted by Regione Emilia Romagna.
This study wants to investigate the whole subject through the method of bibliographic sources.
The literature review has been conduced thanks to the use of main databases, both Italians and foreign. Over 54 eligible articles, 31 were included. 
The documents analyzed were relatively few because this is a quite recent project.
Far-back, the necessity to offer a health care solution to the growing number of people affected by degenerative-chronic diseases has been contemplated. With the purpose to guarantee the health right to all citizens and to promote interventions of reactive medicine is born the idea of “Casa della Salute”.
Relevant health care and patient care regulatory codes have been analyzed, starting from the creation of the National Sanitary System in 1978, passing through the 1992/1993 and 1999 administrative orders, up to the re-organization of the Basic Treatments.
“Casa della Salute” (CdS) is a premises where the social services integrate the sanitary ones, where hospital and territory by way of specialists and experts collaborate together. 
In this structure everyone could get assistance, and a particular attention is given to aged people. Within it, the roles of General Practitioner and nurse are valorized; the latter, operating as Primary Nurse or Case Manager takes entirely care of the patient and coordinates a multi-professional team work, planning a personalized health care. 
Joining the CdS, the citizen will first find a response to his needs, secondarily he will be better oriented facing the plurality of the offered services and, lastly, he will be the main protagonist of his treatment, active and informed.
Thanks to this, the improper accesses to the Emergency Room could be reduced and furthermore, new working possibilities for nurses could be created.
