Abstract
With this thesis I would like to show that at a time when the record is that of chronic diseases and degenerative trend, more and more states need to create the conditions necessary for the man to be well consistent with their limitations, sufferings, large and small disability.
The increase of chronic populations requires more and more focused on the management of the daily management, on education, on prevention, taking charge and the continuity of care. The charities system (GPS, nurses, OSS) aware of their abilities and tecnological availability can provide assistance and territorial home care targeted and effective with significant benefits for people with chronic disease and their families. Hence was born the medicine initiative that gives voice to concepts such as disease management or integrated approach to disease, Care Manager and Self-Management.
