Abstract

In literature, Fluid Management is widely discussed by the scientific community and is recognized as a cornerstone of the care management of patients with diseases such as Heart Failure and Chronic Renal Failure, where control of liquids is an important aspect of care for a good prognosis of the patient himself. In the clinical case presented in this thesis, which concerns one of the possible complications (pulmonary edema) of the coexistence of the two diseases, I got to explain the correlation between these, which in literature is called Cardio-Renal Syndrome (CRS). The goal of patient care with CRS essentially depends on the difficulty of obtaining adequate control of fluid overload avoiding, at the same time, a worsening of renal function. Here is where the nurses, trained, develops highly technical skills in the context of invasive monitoring, management of NIV and ultrafiltration, to identify those issues that can caused the patient's readmission in the hospital and to reduce mortality. Continuous monitoring of the vital functions of the patient with episode of APE and exacerbated CKD by nursing staff, is aimed at recognition of the effectiveness of treatments, a worsening of the clinical and adverse reactions to drugs. Of first priority is the safety of the assisted, paying attention to the management of stress factors, to the comfort of the assisted in the use of adequate equipment, and management of pain with sedation. The need to obtain a continuity in the assumption of drugs and correct lifestyle of the patient makes the nurse to formulate strategies to promote the awareness, by the assisted, of the importance of adherence to therapy and the strict compliance with the prescriptions.
